Project Summary: Curriculum development for a lifestyle medicine, educational treatment program physicians and other clinicians may use to run intensive, therapeutic lifestyle interventions for patients with type 2 diabetes (T2D). The curriculum, designed with a health equity lens, will guide health professionals working in medically underserved communities to support patients in achieving improvements in glucose control, medication deprescription and, when possible, T2D remission. The curriculum will include materials for 1) participants, emphasizing relevance to minority populations, 2) facilitators (clinicians running group programs), and 3) coordinators (support staff responsible for logistics including reimbursement billing).

Curriculum development will involve curation and integration of Full Plate Living curricular resources, existing ACLM educational resources, original scripts and content outlines, and coordination with experts in CPT coding who will be engaged to develop a coding structure for adequate reimbursement. Additionally, target audience representatives will be engaged as context experts to provide review and input on curriculum accessibility, cultural appropriateness, and relevance.

Once developed, this curriculum will be piloted in participants enrolled in the Lifestyle Empowerment Approach for Diabetes Remission (LEADR) Trial, a research study using intensive therapeutic lifestyle intervention for patients living with T2D in the East Central neighborhood of Ogden, UT.
