Grant Opportunity: Whole Person Care Clinical Implementation
Ardmore Institute of Health (AIH) works for a future where healthy environments and lifestyles provide equitable and preferred methods to prevent, treat, and reverse chronic diseases such as obesity, diabetes, and heart disease. Each year, we fund innovative projects that advance our strategic objectives and help make our vision a reality.
AIH is currently seeking to fund Whole Person Care implementation trials within primary care. Please see the example project plan below:
Two-year project that measures select patient outcomes and fiscal impacts
Takes place at a high-performing primary care clinic
Providers are trained in the following:
The HOPE note approach
ACLM’s Lifestyle Medicine and Food as Medicine Essentials Course (5.5 free hours of CME)
AAFP’s Lifestyle Medicine resources
Nudj Health is incorporated within practice
Full Plate Living nutrition programs are implemented to support patients
The Lift Project is provided as wellbeing support for patients (provided at no cost through AIH)
