National Nutrition Month: Start Improving Patient Health Outcomes Today with Full Plate Living
Millions of Americans are facing a future of chronic disease, and 129 million people in the US are living with at least one major chronic disease. Ardmore Institute of Health (AIH) works for a future where healthy environments and lifestyles provide equitable and preferred methods to prevent, treat, and reverse chronic diseases such as obesity, diabetes, and heart disease.
This National Nutrition Month (March), join us in building this future.
Full Plate Living, offered free of cost by AIH, is a research-backed nutrition program that connects individuals to their purpose and teaches them how to add more whole, unprocessed fiber foods to the meals they’re already eating. The Full Plate Living approach is about making small, sustainable changes, and it works!
Celebrate National Nutrition Month with us by utilizing Full Plate Living in your practice:
1. Refer your patients to the Full Plate Living Online Program
Include information in your after visit summary, send patients home with a prescription to learn more or a patient education handout, or display a poster or tabletop display in your practice (we’ll mail them to you).
2. Host a Full Plate Living Facilitated Group Program for your patients and community
Identify a member of your staff to lead a weekly group meeting (in person or virtually) where patients can experience the Full Plate Living program live. We provide all the needed materials and a Facilitator Training.
3. Host a Full Plate Living Shared Medical Appointment Program and bill insurance for your time
We have an 8-session, 4-session, and 1-session program available, and we provide all the needed materials.
As healthcare providers, you have the ability to significantly impact patient health outcomes. By integrating practical, free, evidence-based programs like Full Plate Living into your practice, you empower your patients with the tools and support needed to take control of their health. Together, we can build a healthier future, one plate at a time.
